New Patient Registration

In order to register with the Practice, you will need to complete a registration form. Please ask for one of these at reception or download from on the 'Registering Online' link above. You can either fill this in at the time, or if you need to check your details, you can take it away with you. You will need photographic identification. If you require some help to fill in the form, then please ask a member of staff. It is important to register as we will be unable to get your full medical records until you have done so.

NB If you are on medication, we would ask that you hand in a copy of your repeat medication slip or proof from your previous GP of medications you were taking.  If we do not receive this information with your registration forms we will not be able to issue any medication until we have received verification from your previous GP.  It is best to make sure you  have enough medication to allow us time to receive your medical records. 

If needed, newly registered patients will be invited to have a consultation with a Practice Nurse for a basic health check. This gives you the opportunity to provide us with background medical information. It also provides us with an opportunity to welcome you to the practice, measure your blood pressure and carry out other basic health checks. (Unfortunately due to COVID restrictions this practice has been put on hold)

Please bring a urine sample with you to this appointment. Sample bottles are available from reception.

Please check that you live within our practice boundary as we cannot take patients outwith this area.
